Three Easy Steps:
- Before applying the shipping label to the shipping box, carefully package the items.
Separate the impressions, models and Rx into bags for accurate identification.
Place protective foam or cushioning over the case materials. - Download and print the appropriate RX Order Form below and complete the form. Please sign and date.
- When you are ready to schedule a pick-up either fill out the Schedule a Pick-Up Form below or call us at 707-762-1410.
Include a copy of RxForm with the case and seal the box with packing tape.
